Preregistration Form

E-Mail Preregistration for Seminars

If you wish to preregister, please use the following e-mail form, which will be sent to Prof. Broverman. The full registration form can be found at the registration link. It is expected that the full registration hard copy and payment will be submitted at least one month before the seminar.

Preregistrations and registrations will be acknowledged by e-mail. If you do not receive a reply acknowledging receipt of your preregistration within 2 or 3 days, please contact Prof. Broverman directly either by e-mail at 2brove@rogers.com , or sam@utstat.toronto.edu or by telephone at (416) 966-9111 . The preregistration form below will not be sent if any "required" information is missing. Please let me know if you experience any difficulty with the form.
